| Note: 1. Leveled health care premiums are based on contract salary (not any additional earnings). 2. Doctor co-pays will increase to $9.00 on July 1, 2006. 3. Prescription mail order co-pays will increase on July 1, 2006 to the following: a. Generic from $ 5.00 to $ 6.00 b. Brand from $10.00 to $18.00 (employees retiring during the 2005-06 year or during the term of this extension will pay $15.00 co-pays for one year.)* c. Non-preferred from $25.00 to $35.00 (employees retiring during the 2005-06 year or during the term of this extension will pay $30.00 co-pays for one year.)* *The district has agreed to keep all retirees at the level noted above for one year with respect to brand and non-preferred mail-order drugs. Partial payment for colonoscopy screening for members age 55-64 years of age will be covered beginning July 1, 2006 and will continue through June 30, 2009. The district plan will be amended to include 50% payment up to $500 for 1 screening every 10 years. This benefit will be reviewed annually. Other colonoscopy testing under conditions acceptable to the plan will continue to be covered. (For informational purposes, this benefit is covered under Medicare.) |
